AGREEMENT
I, (We) agree that as team Captain(s) I, (We) have the
authority and the responsibility to organize, administer
discipline, assess and collect team fees incurred for
the participation in playing of the game of Ice Hockey
in the NITTANY HOCKEY LEAGUE. I, (We) shall represent
the team as members of the Board of Governors in determining
League policy, rules and League decisions, including discipline
and suspensions. Each TEAM shall be entitled to one vote.

This agreement is made upon the express condition that
the Nittany Hockey League shall be free from all liabilities
and claims for damages and/or suits for or by reason of
any injury or injuries to any person or persons or property
of any kind whatsoever, whether the person or property
of the Pennsylvania State University, the Nittany Hockey
League, their agents or employees, or any third persons,
from any cause or causes whatsoever while participating
in Nittany Hockey League activities, in or on the premises
or any part thereof, of the Pennsylvania State University's
Ice Rink, whether playing hockey, pre-game or post game
preparation, or as a spectator, or any other activity
in association with the activities of the Nittany Hockey
League during the term of this agreement.
I hereby certify that I,(we) as Team Captain(s), will
not allow or permit anyone whosoever, to play or otherwise
participate in the activities of the Nittany Hockey League
who has not completed a copy of this form. Should I, (We)
fail in this agreement. I, (We) hereby covenant and agree
to indemnify and save harmless the Pennsylvania State
University, the Nittany Hockey League, their agents or
employees, or any third persons, from all liabilities,
charges, expenses (including counsel fees), and costs
on account of or by reason of any such injuries, liabilities,
claims, suits, or losses however occurring or damages
growing out of same.
As evidenced by the signing of this agreement, I, (We)
agree to the details specified herein.
